**31. Stroke Resulting in Permanent Symptoms:** A cerebrovascular incident causing lasting neurological deficits, confirmed by a specialist, with typical clinical symptoms and brain CT/MRI findings. Permanent deficits must last at least 3 months. Exclusions: a. Transient ischemic attacks (TIA) b. Traumatic brain injury c. Vascular disease affecting only the eye/vestibular functions **32. Primary (Idiopathic) Pulmonary Hypertension:** Diagnosis by a Cardiologist or respiratory specialist showing right ventricular enlargement and pulmonary artery pressure over 30 mm Hg via cardiac catheterization. Must have permanent impairment of at least NYHA Class IV. Exclusions include pulmonary hypertension from lung disease or other secondary causes. **33. Permanent Paralysis of Limbs:** Irreversible loss of use of two or more limbs due to brain or spinal cord injury/disease, confirmed by a specialist, with no hope of recovery for over 3 months. **34. Open Heart Replacement or Repair of Heart Valves:** Surgery to replace/repair heart valves due to defects, verified by echocardiography. I need confirmation from a specialist. Excludes catheter-based techniques. **35. Open Chest CABG:** Heart surgery via sternotomy or minimally invasive procedures to correct coronary artery blockage, supported by coronary angiography and confirmed by a cardiologist. Excludes angioplasty and intra-arterial procedures.
### 36. Multiple Sclerosis with Persisting Symptoms: Definite Multiple Sclerosis is diagnosed through typical MRI findings and requires current motor or sensory impairment lasting at least 6 months. Neurological damage from SLE is excluded. ### 37. Motor Neuron Disease with Permanent Symptoms: Motor neuron disease, including spinal muscular atrophy or amyotrophic lateral sclerosis, must be confirmed by a specialist. There must be progressive degeneration and significant neurological impairment for at least 3 months. ### 38. Major Organ/Bone Marrow Transplant: Transplant of a human organ (heart, lung, liver, kidney, pancreas) or bone marrow using stem cells must result from irreversible end-stage failure and be confirmed by a specialist. Exclusions include other stem-cell transplants and islet transplants. ### 39. Major Head Trauma: Permanent neurological deficit from an accidental head injury must be assessed at least 3 months post-accident, supported by imaging. The injury must cause an inability to perform at least three activities of daily living: washing, dressing, transferring, mobility, toileting, and feeding. Spinal cord injury is excluded. ### 40. Benign Brain Tumor: A life-threatening, non-cancerous brain tumor must be confirmed by imaging. It should cause either a permanent neurological deficit lasting at least 90 days or require surgical/radiation treatment. Exclusions include cysts, granulomas, and other specific conditions. |
